Purpose : The latest organ allocation scheme favors heart transplant (HT) in patients with temporary mechanical support devices (tMCS) in United States. However, the traditional approach to avoid tMCS in patients with small body surface area (SBSA) due to higher complication rates has led to inequitable HT. Therefore, we aim to compare the trend in tMCS utilization, HT listing, waitlist mortality, and HT rates in patients with SBSA in a large national database under the new adult heart allocation system compared to the old system. Methods : This study included adult patients listed for HT under the old system (January 1, 2015 - October 17, 2018) and those listed under the new system (October 18, 2018, onward) for the same duration in the Scientific Registry of Transplant Recipients database. They were stratified by BSA (<1.7 m2). The patients were followed until transplant, death, or end of data availability. Waitlist mortality and successful HT were compared between the two groups using a Fine and Gray competing risk regression model adjusted for known risk factors.
