## Background: We conducted the first mortality analysis of COVID-19 and STEMI extrapolated to a nationally representative dataset using machine learning (ML) and propensity score (PS) analysis. Methods: ML-augmented statistical matching was used to unite a single high-volume academic quaternary referral medical center dataset of sequential patients with STEMI and COVID-19 (n = 166) with the largest all payor inpatient dataset in the United States, the NIS for 2016-2018 (n = 101,521,656). ML-augmented PS adjusted regression (ML-PSr) analysis was performed, with PS modified as a disease risk score to account for the likelihood of developing COVID-19. Results: Out of 101,521,822 hospitalizations, 232,645 had primary diagnosis of STEMI (0.23%). In COVID-19 patients, peak STEMI cases occurred from January to March 2020 (36.74%) with significantly higher mortality in the first quarter of the pandemic.
