This case report describes a previously healthy child who experienced recurrent occult pneumococcal bacteremia caused by the same strain of Streptococcus pneumoniae, serotype 6B, at 11 and 15 months of age. Despite appropriate antibiotic treatment, the child showed poor immunological response to the serotype-specific antibodies, indicating a potential maturational immune deficiency. The findings suggest the need for early introduction of pneumococcal conjugate vaccination in Japan to prevent such recurrence
