What is The Pharmacotherapy Review in the Polymorbid Geriatric Patient with Focus on the Risk of Falls and Its Management about?
Drug-related problems connected with falls are one of the main problems of morbidity and mortality in geriatric patients. They significantlydecrease quality of life and increase costs for additional health-care. The analysis of these drug-related problems by a pharmacist is a tool forseeking the effective pharmacotherapy and minimization of all adverse effects connected with a treatment. The goal of this statement was the revision of pharmacotherapy for geriatric patients in aftercare institution and to point out potential drug-related problems which increasethe risk of falls and also other drug-related problems with no relation to the fall. The other part of the work was searching for possible solutionsand eventually performing the interventions which lead to maximization of effect, risk-minimization and to support a patient ́s adherence torational pharmacotherapy.
