The Schilling test is used to evaluate vitamin B12 absorption in patients with vitamin B12 deficiency. It has multiple stages where the patient is given oral and injected doses of radiolabeled vitamin B12 and the amount absorbed is measured in urine over 24 hours. An abnormal result on stage 1 can be followed by stage 2 with additional intrinsic factor to determine if the cause is pernicious anemia. Further stages using antibiotics or pancreatic enzymes may identify other conditions that cause malabsorption
