Background: ACC/AHA guidelines define treatment-resistant hypertension (TRH) as blood pressure over the goal despite optimal doses of 3 antihypertensives including a diuretic, or at the goal while on 4 antihypertensives. Our objective was to identify clinical differences between patients diagnosed with hypertension and those with TRH in the largest inner-city patient population in New Jersey. ## Methods: We investigated all adults diagnosed with hypertension (from July 2020 to July 2022) at the University Hospital Ambulatory Care Center, Newark, New Jersey. Logistic regression analysis was applied to assess the relationship between clinical data and the occurrence of TRH. Variables univariably associated with TRH were entered into a multivariable model to control for confounding. Results: From a total of 3588 patients with hypertension, 11% (n=378) of them were identified with TRH. After multivariable adjustments, patients who were older (OR per 1 year increment [95% CI]: 1.04 [1.02-1.07], p<0.001), diabetic (OR 2.10 [1.54-2.87], p<0.001), obese (OR 2.09 [1.66-2.63], p<0.001), and had a stroke (OR 1.60 [1.16-2.2], p=0.004) had significantly higher odds of having TRH.
