Background: Tricuspid Atresia (TA) is a complex congenital heart disease that requires multiple procedures. Due to limited training and technology, transcatheter therapies (TcT) are used infrequently in low-and-middle income countries. Case: A 4-year-old female diagnosed with TA at age 6 months and irregular follow up, presented with an O2sat of 89%. An echocardiogram showed TA with a restrictive ventricular septal defect and normally related great vessels (Figure). Decision-making: Cardiac catheterization revealed a PVR of 1.6 WU and a MPA pressure of 18 mmHg. Along with a Glenn, a 20 mm Gore-Tex conduit with a large fenestration was connected from IVC to RPA. A pericardial membrane was sutured inside the conduit, precluding IVC flow into the lungs. The patient was discharged on sildenafil. Six months later, catheterization showed a PVR of 1.1 WU and MPA pressure of 10 mmHg. At this time, the membrane was crossed with a needle and dilated with a 15 mm balloon. A 4.5 cm covered stent was delivered further opening the membrane and closing the fenestration.
