This case report discusses two patients with metastatic papillary thyroid cancer (PTC) who experienced rapid worsening of malignant pleural effusion (MPE) during L-thyroxine (LT4) withdrawal for radioactive iodine (RAI) therapy. Both patients had extensive metastasis and deteriorated significantly, ultimately leading to their deaths due to disease progression. The findings suggest that LT4 withdrawal may adversely affect PTC patients, particularly those with MPE.
