This study evaluated the use of minimally invasive distal metatarsal osteotomy (MIDMO) without internal fixation for the treatment of primary metatarsalgia in 4 patients. MIDMO involves making small incisions to perform oblique osteotomies of the second, third, and fourth metatarsals using a burr. This corrected imbalances and relieved pressure without requiring internal fixation. All osteotomies healed within 4 months with no major complications.
