This case report discusses the treatment of a 19-year-old woman with a complex distal comminuted femoral fracture and subsequent leg length discrepancy. The patient underwent multiple surgeries, including the use of an intramedullary skeletal kinetic distractor (ISKD) to successfully address a 6.5 cm leg length shortening.
