As a contributor to the Tokyo Guidelines, I read with interest "Evaluating the Relevance of the 2013 Tokyo Guidelines for the Diagnosis and Management of Cholecystitis" in the July issue of the Journal. 1 In this article, it was concluded that "The TG13 diagnostic criteria lack sensitivity and missed more than half of the patients with acute cholecystitis. . . ." When attempting to validate the Tokyo Guidelines for acute cholecystitis, it is essential that all patients do actually have acute cholecystitis. There are strong indicators that this criterion was unmet.
