What is Wilms' Tumor with Pulmonary Nodules Persisting After Chemotherapy and Radiation about?

This Tumor Board is convened in order to discuss an unusual problem and to reach a management decision. The difficulty we face is that there are ambiguous, persistent findings noted on follow-up imaging studies of a child under treatment for Wilms' tumor. The patient is a five-year-old girl who presented with a 1 week history of hematuria which was not responsive to oral antibiotic treatment. Upon admission to Rainbow Babies and Childrens Hospital, a large right flank mass was noted. There was no prior history of dysuria, frequency, fevers, nausea, vomiting, or back pain. There was no recent change in activity level, anorexia, or weight loss. On the morning of hospital admission, she experienced back and flank pain, and emesis, which resolved with intravenous hydration. Physical examination revealed a well-appearing little girl with mild pallor in no distress. Vital signs, including blood pressure, were within normal limits. Physical examination was significant for a right sided abdominal mass measuring approximately 10 X 15 cm. The lungs were clear with equal breath sounds bilaterally.
