This study evaluates the safety and outcomes of intravenous thrombolysis (IVT) in acute stroke patients treated beyond the standard 4.5-hour time window using computed tomography perfusion imaging. A total of 70 patients were analyzed, with 34.3% undergoing mechanical thrombectomy and a 5.7% incidence of symptomatic intracerebral hemorrhage. The findings suggest that IVT based on CTP imaging can be a safe option for patients in an extended time window, warranting further clinical trials for validation.
