A 22-year-old female presented with progressive muscle wasting in her face and arms over the past 2 years. Examination revealed muscle wasting in her facial muscles and deltoid regions bilaterally. Investigations showed elevated ESR and biopsy results suggestive of granulomatous inflammation. The differential diagnoses included facioscapulohumeral muscular dystrophy, limb girdle muscular dystrophy, and upper cervical cord lesion due to disseminated tuberculosis.
