What is False Positive High Sensitive Troponin Elevation From Immune Mediated Necrotizing Myopathy about?

Background: While cardiac troponins are the most specific marker for myocardial injury, they are not specific for myocardial infarction. Current 4th Universal Definition for acute myocardial infarct, requires a rise and/or fall (with >20% change) of cardiac troponin (cTn) above the 99th percentile of the upper reference limit and with at least-1 ischemic criteria. Non-ischemic troponin elevation occur from multiple cardiac and systemic disorders with added diagnostic and prognostic value. A less frequent cause for unexplained troponin elevation is that related to false-positive (FP) analytic errors. We present a patient with FP troponin elevation related to a statin associated immune mediated necrotizing myopathy (IMNM). Case: A 63-year-old male with history of arterial hypertension, diabetes mellitus type II, and a recently diagnosed IMNM, presented to the ED after an episode of palpitations, dizziness, and low blood pressures, with an impression of medication induced hypotension. Laboratories showed elevated hs-cTnT values of 444, 429, and 466 (abnormal cut-off value of >22 ng/l), at 0, 1 and 3 hours, respectively, with a non-specific delta zig-zag pattern and with elevated CPK l
