Subcutaneous low-dose heparin and graduated compression stockings are established methods of reducing the incidence of postoperative deep vein thrombosis (DVT). The adequacy of implementation of DVT prophylactic policies is unknown. The aims of this study were to determine how closely DVT prophylactic policies are adhered to in routine general surgical practice, to identify reasons for policy violations and to assess the effects of policy modification. All 80 adult general surgical inpatients on one weekday were studied. Nursing and junior medical staff were not forewarned of the study. Patient details, the surgery undertaken and risk factors for DVT were recorded. Patients were allocated to low, moderate or high risk groups. Details of administered DVT prophylaxis was recorded. Of 60 patients who had undergone an operation at the time of the study, 30 (50%) did not receive DVT prophylaxis according to the written policy of the relevant consultant.
