Background: Excessive daytime sleepiness (EDS), a common symptom complicating patients with obstructive sleep apnea (OSA), is closely associated with the development of acute coronary syndrome (ACS), but its effect on prognostic value of OSA is uncertain. We aimed to investigate the association of OSA with long-term cardiovascular events in ASC patients with or without EDS. Methods: From June 2015 to Jan 2020, consecutive eligible patients with ACS underwent cardiorespiratory polygraphy during hospitalization. OSA was defined as an apnea-hypopnea index (AHI) ≥15 events/h. The Epworth Sleepiness Scale was completed before cardiorespiratory polygraphy, and EDS was defined as a score ≥ 10. The primary outcome was major adverse cardiovascular and cerebrovascular events (MACCE), including a composite of cardiovascular death, myocardial infarction, stroke, ischemia driven revascularization, or hospitalization for unstable angina or heart failure. Results: A total of 1154 subjects with ACS were finally enrolled. Mean patients age was 56.8 ± 10.5 years, and 83.8% were male.
