Renal flare and vaccination failure: 4 case reports In a prospective study of 35 patients aged 12-18 years with SLE and lupus nephritis who had renal remission, 4 patients [exact ages not stated; sexes not stated] were described, who developed renal flare or exhibited vaccination failure following vaccination with tozinameran for immunisation against COVID-19 [dosages and routes not stated; not all outcomes stated].All the patients had SLE and lupus nephritis, and were in renal remission. All patients received two doses of tozinameran [BNT162b2] mRNA vaccination and were on prednisolone prior to vaccination. However, 3 patients had renal flare within 3 months post vaccination. One patient showed renal flare manifested as mild proteinuria and mild microscopic haematuria which improved after increase in prednisolone dose. Two patients developed severe renal flare, one with class V lupus nephritis and other with lupus nephritis and thrombotic microangiopathy. Additionally, one patient developed COVID-19 at 2 months post vaccination, indicating vaccination failure.
