This study identifies clinicopathologic factors associated with the early detection of clinically significant prostate cancer (csPCa) following a negative MRI-informed biopsy. Key findings indicate that higher PSA density and persistent PI-RADS 4 to 5 lesions on repeat MRI significantly increase the risk of csPCa, while inflammation on initial biopsy appears to lower that risk. The results suggest that patients with these risk factors may benefit from earlier follow-up imaging or biopsy to improve detection
