What is Lupus Myocarditis: A Rare Presentation about?

Background: Lupus myocarditis (LM) has a prevalence of 8% and presents with tachycardia, ST wave abnormalities, cardiomegaly, and congestive heart failure. Case: A 27 year old male with epilepsy and systemic lupus erythematosus presented with acute onset fever, positional chest pain, dyspnea on exertion, anorexia, nausea, and vomiting for three days. Physical exam was notable for hyperthermia (38.9 C), tachycardia (114 bpm), tachypnea (25 breaths/min), hypoxia requiring 3L of oxygen, bilateral malar erythematous rash, and irregularly irregular rhythm on cardiac auscultation. Electrocardiogram demonstrated atrial fibrillation with rapid ventricular response and 2mm ST elevations in the lateral leads. Laboratory studies were positive for hypokalemia (2.8 mmol/L), serial hypertroponinemia (36.943 -> 38.961 ng/mL), positive anti-nuclear antibodies (1:320), and hypocomplementemia (C3: 86 mg/dL and C4: 9 mg/dL). Chest radiograph showed early interstitial edema. Decision-making: Emergent cardiac catheterization revealed normal coronary arteries on angiogram and elevated left ventricular enddiastolic pressure (25mmhg). Right ventricular endocardial biopsy confirmed the diagnosis of active
