intensity modulated radiotherapy (IMRT), which was optimized based on single photon emission photography (SPECT) lung perfusion imaging in unresectable Stage III non-small cell lung cancer (NSCLC) and to investigate the safety, efficacy. Materials/Methods: Patients with Stage III NSCLC were treated every 21 days. The regimen was cisplatin 25mg/m2 on Days 1-3, 22-24 with escalating pemetrexed doses (400mg/m2/500mg/m2). The trial included concurrent LCAF IMRT (first phase: 40 Gy/ 20f/2 Gy; second phase: 1.4 Gy twice a day; total dose was determined by lung V20=35%). The gross tumor volumes were delineated according to PET-CT imaging. In order to spare functional lung, SPECT lung perfusion imaging was employed to optimize IMRT plans. Granulocyte Colony stimulating factor (GCSF) and supportive cares were allowed during concurrent chemoradiotherapy while medically needed. Dose limited toxicities of the study were defined as following according to CTC 3: Grade 4 white blood depression or neutropenia or febrile neutropenia; Grade 4 thrombocytopenia; Grade 3 or 4 pulmonary or esophageal toxicity; any other Grade 3 or 4 nonhematologic toxicity.
