Low-dose rate brachytherapy has demonstrated efficacy in the treatment of extremity soft tissue sarcoma (ESTS). The use of high dose rate brachytherapy (HDRBT) has not been well characterized, however. This study was performed to evaluate the efficacy and toxicity of perioperative HDRBT as a component of treatment of ESTS. Materials/Methods: The records of 51 consecutive patients (27 females, 24 males) treated with HDRBT for ESTS from June 1998 through January 2005 were reviewed. Median age at treatment was 55.7 years (range, 18 to 86 years). All patients underwent limb-salvage surgery with interstitial tube placement prior to HDRBT. While no patients had positive margins, marginal excision occurred in 24 patients (47%). Fifty patients received external beam radiation therapy (EBRT), 45-50.4 Gy, either preoperatively (76%) or postoperatively (22%). Three patients were treated for recurrent disease. Brachytherapy was delivered over two or more days starting an average of 4 days after surgery (range, 1-7 days) with an interfraction interval of at least 6 hours.
