Mixed features are highly prevalent and have been reported in a percentage of depressive episodes ranging from 46.4% to 73.1% in patients with BD, and from 7.6% to 48.7% in patients with MDD ( Shim et al., 2018 ). Mixed features are correlated with a more severe clinical course, including earlier onset, longer time to remission, poorer response to treatment, more recurrences, and higher risk of suicide, compared with affective episodes that occur without mixed features . DSM-5, allows a diagnosis of major depressive episode with mixed features specifier, for both bipolar (BD) and major depressive disorder (MDD). However, no pharmacological treatment is specifically approved for MDD (and BD) with MFS.In this paper, we argue that patients with major depressive disorder (MDD) who concomitantly experience mixed features and/or any symptom of 'activation' should be: 1) initially treated with an antimanic agent (mood stabilizer and/or an antipsychotic); 2) considered for adjunctive treatment with an antidepressant that does not increase the levels of norepinephrine or dopamine, such as trazodone or citalopram; 3) considered for discontinuation of the antimanic agent, once the acute episo
