Introduction: Early directed therapies for sepsis have been reported to improve patient outcomes. Most sepsis diagnostic strategies utilize systemic inflammatory response syndrome (SIRS) criteria, which have been criticized as non-specific for infection. Given the issues of ED overcrowding and potential pandemics, diagnostic approaches to rapidly identify patients with sepsis are needed. Study Objective: To assess the utility of emergency department triage vital signs to predict infection in patients with evidence of SIRS. Methods: Analysis of prospectively collected information on all patients presenting to a level I academic trauma center with 2 or more criteria for systemic inflammatory response syndrome (SIRS) and suspected infection. Triage vital signs included systolic blood pressure (SBP), diastolic blood pressure (DBP), pulse pressure (PP), mean arterial pressure (MAP), heart rate (HR), respiratory rate (RR), temperature and calculated shock index (HR/SBP).
