What is Brash Syndrome: A Systemic Review about?
Background: The pathophysiology of Bradycardia-Renal Failure-Atrioventricular Nodal Blockade-Shock-Hyperkalemia (BRASH) syndrome involves acute renal injury leading to ineffective clearance of AV nodal agents and potassium. Theoretically, the synergy between AV nodal blockade and hyperkalemia results in circulatory collapse at less-than-expected doses of both. This study aims to characterize the presentation of BRASH and its risk factors. Methods: This is a systematic review of all reported cases of BRASH until February 2022 using the EMBASE, CINAHL and Web of Science databases. Patient demographics, comorbidities, presenting symptoms, vital signs, EKG findings, AV-nodal blocking agents, presenting serum potassium and creatinine, therapeutic strategies, and in-hospital mortality were extracted and analyzed using descriptive statistics. Results: Seventy cases were included in our study. The average age and Charlson Comorbidity Index at presentation was 69 years and 3.8 respectively -hypertension (71%), diabetes mellitus (48%), and chronic kidney disease (44%) were prevalent. The most frequent chief complaint was fatigue or syncope (49%). More than half of all patients presented with
