respectively. With 2 mm uniform margins, the percentage of time that the prostate moved outside was 0.96+/-0.98, 19.3+/-12.9 and 17.54+/-12.61, respectively. Individual treatment margins derived by using the mathematical model varied significantly from patient to patient with ranges as follows (in mm): +x (2 to 3), -x (2 to 3), +y (2 to 6), -y (3 to 6), + z (2 to 6), and -z (4 to 6). The percentage of time the prostate moved outside the individual patient margins based on the model was 0.86+/-1.07, 2.56+/-3.65, and 4.37+/-4.24, respectively. In one patient, as many as 10% of the motions in the y and 5.62% in z direction exceeded treatment margins using 6/4 mm margins and was similar to that predicted by the model. However, it was worse when using a uniform 2 mm margin in which 36.1% (y) and 36.4% (z) of the movements exceeded the pre-defined margins. Conclusions: We do not advocate the use of 2 mm margins. Individualized predictive treatment margins provide a better approach than the conventional fixed margin approach. The new approach may also translate into individualizing toxicity based on patient specific prostate motion during treatment.
