What is Midazolam: Lack of efficacy: case report about?

Lack of efficacy: case report A 48-year-old man exhibited lack of efficacy during treatment with midazolam for generalised convulsions. The man administered thiamethoxam pesticide and developed nausea and vomiting of gastric contents. He presented to a local hospital with systemic convulsions, limb spasms, shortness of breath, unconsciousness and decreased oxygen saturation due to acute pesticide poisoning. He was hospitalised and received treatment with sodium valproate, omeprazole, methylprednisolone and fluid replacement for pesticide poisoning. He underwent endotracheal intubation for respiratory support and gastric lavage treatment. He received treatment with propofol for sedation and the stomach was washed with water. Thereafter, he was transferred to the emergency room of another hospital for further treatment. He started receiving treatment with continuous midazolam [route not stated] 0.2 mg/kg/hour through micropump for convulsions. However, he still had generalised convulsions with one episode lasting for 10 seconds, indicating lack of efficacy with midazolam. Thereafter, he received sodium chloride [saline], montmorillonite and mannitol for treatment of acute poisoning.
