Mr. Iqbal, age 56, presented with a history of backache and pain radiating to the lower limb. An MRI of the lumbar spine found straightening of the lumbar spine likely due to muscle spasm. There was multilevel disc desiccation and bulge at L4/L5 and L5/S1 indenting the ventral thecal sac and narrowing the neural foramina. A central disc extrusion was seen at L2/L3 protruding 9mm and causing lateral recess and thecal sac stenosis, resulting in cauda equina compression. No other abnormalities were noted.
