A hiatal hernia occurs when the upper part of the stomach pushes through the diaphragm into the chest cavity. Contributing factors include weight lifting, strenuous activity, constipation, pregnancy, and tight clothing around the abdomen. This causes permanent shortening of the esophagus and a decrease in lower esophageal pressure, leading the gastroesophageal sphincter to become incompetent and allow reflux and regurgitation of stomach acid, resulting in inflammation and scarring of the esophagus.
