This case report describes a rare case of primary hemorrhage in the cavum veli interpositi, an abnormal enlargement of the velum interpositum, as the source of intraventricular hemorrhage in a 17-year-old woman. Diagnostic imaging showed blood predominantly in the cavum veli interpositi with no apparent vascular lesion.
