What is One Year Outcomes Of Cto Vs Non-cto Femoropopliteal Lesions Treated With Atherectomy Followed By Drug-coated Balloon Angioplasty about?

Background: The endovascular management of chronic total occlusions (CTOs) of the femoropopliteal (FP) segment is technically challenging and associated with increased rates of treatment failure and complications. The long-term patency of CTOs of the FP segment treated with contemporary tools, such as atherectomy and drug-eluting technology, is not well studied. Methods: We performed a retrospective, single-center analysis of 60 patients with FP disease treated with either directional or orbital atherectomy followed by angioplasty with paclitaxel drug-coated balloon (DCB). 26 patients had CTOs versus 34 who were treated for non-CTOs. All patients underwent close clinical and imaging evaluation for 1 year to identify evidence of target lesion restenosis. Results: Baseline demographic and clinical characteristics were similar between the two groups other than ankle-brachial indices (ABI), which were worse among patients with CTOs (0.73 ± 0.11 vs 0.88 ± 0.14, p<0.001). The treated lesion length was longer in the CTO group compared to the non-CTO group, (168 ± 58.5 mm vs 130 ± 41.4 mm, p=0.011). Post-atherectomy, lesions in the CTO group demonstrated a numerically greater percent steno
