## Insulin Hypoglycaemia following off-label use: case report In a prospective case series conducted from July 2017 to January 2019,, a 48-year-old man was described, who developed hypoglycaemia following off-label treatment with insulin for hypertriglyceridaemia-associated acute pancreatitis (HTGAP). The smoker man, who had type 2 diabetes mellitus and hypertriglyceridaemia, had been receiving unspecified HMG-CoAreductase-inhibitors [statin], gemfibrozil and ezetimibe. He developed deteriorating hypoxia and tachycardia, which led an ICU admission. He was admitted and diagnosed with HTGAP. For the management of HTGAP, he was maintained nil by mouth (fasting). Additionally, he was initiated on unspecified IV fluids and off-label treatment with IV infusion of insulin at 4 units/h (0.05 units/kg/h) which was subsequently increased to 8 units/h (0.1 units/kg/h). Concomitantly, he received enoxaparin sodium for deep venous thrombosis prophylaxis. Glucose [dextrose] was titrated separately after the blood glucose levels were <10 mmol/L, with an aim for blood glucose levels between 5-10 mmol/L.
