48 subjects either with duodenal ulcer (DU) (23) or with an operated stomach following DU (25) were given 0.2 U/kg insulin i.v. This dose was shown to decrease blood glucose by more than 50%. Gastric secretion was continuously collected by aspiration into 15-min samples (basal: 120 min; stimulated: 120 min). Plasma gastrin levels were determined according to the method of Yalow and Berson, the assay having a sensitivity of around 2 pg/ml (sampling every 30 min before insulin injection, and at 15, 45, 75, 105 min). (1) In the 23 DU patients, the mean basal plasma gastrin (individually calculated on the mean of the 5 preinsulin samples) was significantly different from the mean of the post insulin peaks (m ean±SE: 37.08± 10.26 vs. 58.95± 15.9 pg/ml; p <0.001). The correlation coefficient between basal plasma gastrin and the maximal absolute increase calculated from the peak gastrin value was found to be: r = 0.86 (p 0.001). Plasma gastrin elevations greater than 40% were observed in 15 out of the 23 patients studied.
