Cardiac endothelial inflammation leading to atrial fibrillation: case report A 55-year-old woman developed cardiac endothelial inflammation leading to atrial fibrillation (AF) following vaccination with elasomeran.The woman with a history of paroxysmal AF, hypertension, and mild mitral valve prolapse ( ) presented to hospital with complaints of palpitations, acute chest tightness and dyspnoea. Three days before presentation, she had received her first dose of elasomeran [mRNA-1273; route and dosage not stated]. Initially, she noted palpitations, which did not resolve within 24 hours; subsequently, she started experiencing acute chest tightness and dyspnoea. She had no known allergies, had received all standard vaccinations, and had no previous history of any adverse reactions. Upon admission, she was hemodynamically stable and afebrile. Electrocardiographic findings suggested AF. Laboratory examinations showed a high-sensitivity troponin level of 0.045 ng/mL. Within 24 hours, the troponin level peaked to 0.086 ng/mL. Chest X-ray revealed an enlarged cardio-mediastinal silhouette. Echocardiogram indicated normal left ventricular ejection fraction (63%), moderate-to-severe tricuspid
