What is Multimodal Imaging Evaluation And Treatment Of A Catecholamine-induced Cardiomyopathy about?

Background: Catecholamine-induced cardiomyopathy occurs in roughly 8-11% of patients with pheochromocytoma. Case: A 67-year-old female presented with chief complaint of syncope. Upon arrival she was noted to have labile blood pressure, initially 187/129, dropping to 76/48 by the time she arrived on the medical floor. A transthoracic echocardiogram revealed ejection fraction of 45% with diffuse apical akinesia, as well as mid-ventricular and basilar hyperkinesia. Apical to basal intracavitary gradients demonstrated a progressive increase and an incidental mass was identified in the retroperitoneum, medial to the left kidney on CT scan (Figure ). Cardiac MRI was obtained for further evaluation and normetanephrine ultimately returned at 6,096 ug per 24 hours, confirming a pheochromocytoma. Decision-making: In the current era, emphasis should be placed on early clinical recognition and multi-modal imaging evaluation as part of the diagnostic evaluation of these rare metabolic cardiomyopathies. Alpha followed by beta adrenergic antagonism remains the cornerstone of therapy in the medical management of pheochromocytomas. A multi-disciplinary approach should be emphasized and in severe ca
