What is Critical Limb Ischemia From Rheumatic Mitral Stenosis about?
Background: Acute limb ischemia has devastating consequences and threatens limb viability. The majority of cases are due to arterial occlusion from thrombosis of an existing diseased artery. For patients without native diseased arteries, emboli originating from the heart are the most common. Few cases have been described in the literature documenting rheumatic heart disease associated with valvular atrial fibrillation leading to acute limb ischemia. Case: A 30-year-old previously healthy African American male presented acutely with bilateral lower extremity pain and numbness. Given concerns for limb ischemia, he underwent emergent bilateral lower extremity computed tomography angiography, which showed extensive thrombosis spanning the right common iliac artery down to the right popliteal artery. Electrocardiogram revealed atrial fibrillation. Transesophageal echocardiography (TEE) showed a 1.2 cm echogenic left atrial appendage mass, commissural fusion, and leaflet thickening suggestive of rheumatic mitral stenosis.
