Background: Cardiac infiltration by hematological malignancies is uncommon. Diagnosis of cardiac mass due to chronic lymphocytic leukemia (CLL) B cell type relapse with non-diffuse bone marrow involvement is extremely rare. Case: 51 yr-old male with History of CLL-in remission since 2006-presented with B symptoms and exertional dyspnea. He was vitally stable. Initial Labs showed leucopenia and thrombocytopenia. CT chest revealed mediastinal and supraclavicular lymph nodes (LN) with 8.8 cm mass occupying the majority of the right (RA) and extending to the right ventricle (RV). A,B Decision-making: Cardiac MRI showed a 10 x 7.6 cm mass in RA and RV extending into inferior vena cava and coronary sinus, infiltrating interatrial septum, tricuspid valve, RV inferior wall and pericardium with 21 mm of pericardial infusion inferiorly. C,D Trans-esophageal echocardiography (TEE) confirmed the mass and excluded tamponade. Bone marrow was normo-cellular in biopsy .
