Purpose/Objective(s): Carcinoembryonic antigen (CEA) values are used to monitor rectal cancer patients after curative treatment. However, there is no consensus on how to best use these values to predict and detect recurrence. We evaluated the prognostic impact of post-treatment CEA values, including their role in the setting of an uncertain surveillance CT scan. Materials/Methods: Patients in this study underwent curative-intent surgery for rectal cancer. Surveillance CEA and CT scans were obtained approximately every 3-6 months, and every 6-12 months, respectively. Analysis of CEA values included the pre-treatment value, the first post-operative value, nadir value, max value over the first year, and max value over all follow-up. Patients were stratified by the median value, and freedom from failure (FFF) was evaluated using Kaplan-Meier log-rank test.
