Norepinephrine is recommended as the first-line vasopressor for raising blood pressure in patients with septic shock. Vasopressin can be added to norepinephrine to further increase blood pressure or decrease the norepinephrine dose. Epinephrine is recommended as a second-line agent if norepinephrine alone is not enough. Dopamine is almost never used due to safety concerns, except in rare cases of bradycardia. Phenylephrine is not recommended except as a last resort salvage therapy.
