Mr. Latif, a 68-year-old Malay man, presented with right abdominal pain, fever, and hematuria. Upon examination, he had tenderness over his right lumbar region and illiac fossa with costovertebral angle tenderness. Bloodwork showed elevated white blood cells. A provisional diagnosis of acute pyelonephritis was made given his symptoms of right flank pain, fever, and hematuria. Differential diagnoses considered included renal colic, cholecystitis, appendicitis, and pancreatitis but were deemed less likely.
