A 58-year-old postmenopausal woman presented with a painless lump in her left breast that had gradually increased in size over one year along with nipple retraction and ulceration for the past 4 months. On examination, a 6x5 cm firm lump was found in the left breast along with an irregular skin ulceration and palpable left supraclavicular lymph nodes but no axillary lymph nodes. A provisional diagnosis of Stage T4N3M1 carcinoma of the left breast was made pending further diagnostic investigations and biopsy
