Conclusions: Despite a demonstrably higher combat tempo and injury severity as evidenced by the rates of battle casualties, killed-in-action and evacuation, the survival rate at the study facility was significantly better than published aggregate rates for OIF Phases I and II. We attribute this finding to the presence of trained emergency medicine specialists, and to the introduction of EMS medical direction principles to the setting of combat casualty care. Prospective studies designed to validate and implement an emergency medicine-based combat casualty care model, and evaluation of the effect of adding a greater range of advanced life support equipment to forward medical units, is warranted.
