An 84-year-old woman had a dermal cyst on her left temple that was lined by two layers of cells: an outer layer of flattened myoepithelial cells and an inner layer of tall columnar cells containing red cytoplasm and basally placed nuclei. This was identified as an uncommon apocrine hidrocystoma, a solitary dermal cyst on the head or neck derived from an apocrine duct and characterized by its two-layer cell lining and presence of decapitation secretion.
