Soft tissue sarcomas are rare malignancies and encompass over 50-70 histological subtypes. For patients with sarcomas in the retroperitoneum, there is increasing recognition that treatment, including the approach to surgery, should be guided by histologic subtype. 1,2 In this anatomic location, the most common histologic subtype is liposarcoma, including well differentiated and dedifferentiated disease. Leiomyosarcoma is the second most common histologic subtype and clearly has a distinct disease biology in comparison to liposarcoma. The central issue remains how to effectively incorporate our understanding of disease biology into clinical decision-making. In this issue of the Journal of Surgical Oncology, Ikoma et al 3 report a retrospective series of patients with retroperitoneal leiomyosarcoma (RP LMS) seen at the University of Texas MD Anderson Cancer Center (MDACC). The data reaffirms that for this histologic subtype, resection is frequently complex with 77% of cases requiring concomitant organ resection and 44%, great vessel resection.
