What is Letter to the Editors about?
There is little question that the criteria for liver transplantation vary widely throughout the United States. Given the shortage of organs for transplantation and the associated disputes over organ allocation, it is essential that more uniform criteria be used by centers to select patients for liver transplantation. The article in Liver Transplantation and Surgery, by Lucey et al (3:628-637, 1997) is a step in this direction. However, we would like to echo the comment in the accompanying editorial by Dr Shaw that some of the criteria may be ''too minimal.'' We believe this caveat applies specifically to the statement ''cirrhotic patients who have experienced gastrointestinal bleeding caused by portal hypertension . . . would meet the minimal criteria irrespective of their Child-Pugh score.'' If this recommendation is followed, we believe that patients will receive transplant unnecessarily. We believe that variceal bleeding is not an indication for transplantation in patients with well-preserved liver function.
