What is Long Term Outcomes With Use Of Sacubitril/valsartan In Adults With Congenital Heart Disease about?
Background: Heart failure (HF) is a growing burden in adults with congenital heart disease (ACHD). Use of sacubitril/valsartan is beneficial in HF patients with structurally normal hearts, but little is known about tolerability in the ACHD population. We hypothesize that sacubitril/valsartan will be safely tolerated without substantial risk of hypotension or acute kidney injury (AKI). Methods: This was a retrospective review of 35 ACHD patients at our institution on sacubitril/valsartan between Jan. 2017 -Jun. 2022. Demographics, systemic ventricle function by echo, peak exercise oxygen consumption (pVO 2 ), creatinine (Cr) and New York Heart Association (NYHA) functional class were collected. Continuous variables are presented as means and compared with student t-test. Results: The study population was mostly male (71%) with a median age of 35 years (range 25-84). Median follow-up on sacubitril/ valsartan was 24 months (range 1-46). 16 patients (45.7%) improved their NYHA functional class or peak VO2 after starting sacubitril/ valsartan. There was a trend towards improved ventricular function in patients with systemic LV (35.4% to 40%, p = 0.2) and systemic RV. No patients develop
