were recruited. They were dialyzed using dialysate bicarbonate concentration of 35 meq/l according to the unit protocols. Following informed written consent, a blood sample was drawn through AVF just before and soon after the HD to analyze SBC. A total of 135 HD events in 45 patients were analyzed. Severe acidosis was defined as SBC less than 17 mmol/l. The ethical approval was obtained from hospital ethical review committee. Results: The mean age of the cohort was 50 years. Diabetes mellitus (40%) and hypertension (35%) were the main causes of end stage kidney disease. The mean hemoglobin and phosphate levels were 9.23 g/l and 3.8 mmol/l respectively. The urea reduction ratio was over 70 % in all patients. Blood flow rate range was 213-303 ml/min (mean-288 ml/ min), and ultrafiltration rate was between 1.7-5.0 L (mean-3.8L). Just after HD, SBC concentrations were normal in all events. However, the depletion of SBC causing the lowest value was recorded on 4 th day (18.61AE4.14 mmol/l) (p= 0.046). SBC of less than 17 mmol/l was found in nearly two third of (62%) all events (40.5% in D4 and 21.5% in D3) (p=0.018). The mean bicarbonate gain during HD was significantly higher in day 4
