This case report discusses two successful applications of miniature pulpotomy (MP) using calcium-enriched mixture (CEM) for treating symptomatic mature molars with irreversible pulpitis and apical periodontitis. Both cases demonstrated resolution of pain, maintained vitality, and showed healing of apical lesions over an 18-month follow-up period. The findings suggest that MP with CEM may be a viable alternative to traditional root canal treatment for certain cases of irreversible pulpitis.
