Background: LVAD outflow graft obstruction (OGO) from extrinsic, fibrothrombotic compression or internal thrombosis can obstruct flow, causing symptomatic hypotension. Few catheter-based interventions have treated OGO. Case: An elderly man with ICM, stage D HFrEF, and 5-year-old HM3 presented with AMS and low MAPs. Speed, flow, power, and PI were stable. There was no hemolysis. INR was therapeutic. OGO was suspected. CT-A showed an eccentric, hypodense, >50%, proximal outflow graft (OG) lesion, identified as internal thrombus. Decision-making: IVUS ruled out internal thrombus. 2 self-expanding BMS and 1 covered stent were placed with Sentinel protection. Clopidogrel was given for 1 month. Repeat CT-A confirmed patency. Conclusion: Successful graftoplasty of a HM3 OGO improved MAPS and symptoms.
