What is Myasthenia gravis about?

Myasthenia gravis is a neuromuscular transmission disorder that is immune-mediated. Antibodies are directed toward proteins of the neuromuscular junction, primarily the nicotinic acetylcholine receptor (AChR) and the muscle-specific kinase. The antibodies, which are produced by autoreactive B cells and regulated by T-cell signaling, mediate injury by impairing antigen function, removal of the target protein, or complement destruction of postsynaptic membrane. The autoimmune attack leads to a reduction of functional AChR at the postsynaptic muscle surface and weakness with a characteristic worsening with prolonged activity. Electrophysiological tests demonstrate a decremental response to repetitive stimulation. The clinical phenotypes vary based on distribution of muscle involvement with some patients having weakness of ocular muscles producing double vision and ptosis while others have severe, generalized weakness, which required treatment with mechanical ventilation. Therapies for MG are primarily focused on moderating antibody-mediated injury.
